George A. Wade, MD
George A. Wade, MD
Dr. Wade is the founder of Idaho Sports Medicine Institute. He is a graduate of the University of Michigan, where he was an All-American in track. In 1978 after completing a fellowship in sports medicine at the Kerlan-Jobe Clinic in California, Dr. Wade came to Boise realizing a lifelong dream of starting a sports medicine clinic. He has served as the team physician for Boise State University since 1979. Dr. Wade is also a team physician for Boise Hawks Baseball and area high schools. Dr. Wade is an advocate for youth and athletics. He is a backcountry skier, rafter and kayaker and enjoys spending time with his son, who is an outstanding kayaker.

Scot B. Scheffel, MD
Dr. Scheffel graduated from the University of Nevada, Reno, with high distinction. He joined Drs. Wade and Lewis in September of 2000 to pursue the treatment of medical and orthopedic sports related conditions. He is a team physician for Boise State University, Boise Hawks Baseball and Centennial High School. Dr. Scheffel is an Associate Clinical Professor for the University of Washington. He is also the Director for the primary care sports medicine fellowship and oversees the sports medicine training for local family medicine residents. He enjoys golfing, skiing, mountain biking, and spending time with his family.

Dr. Gustavel is a Boise native and Borah High School graduate. He received his undergraduate degree from the University of Idaho, where he played basketball and graduated summa cum laude. Dr. Gustavel joined ISMI in September of 2002. He serves as a team physician for Boise State University, Boise Hawks Baseball and Boise High School. Dr. Gustavel enjoys working out, music, snowboarding and spending time with his wife and two daughters.

Jennifer R. Miller, MD
Dr. Miller, a Boise native, was a volleyball, basketball and track athlete at Boise High. She joined the ISMI team in 2004. She is a team physician for Boise State University, Boise Hawks Baseball and Borah High School. She serves on the Boise School District sports medicine curriculum committee, the Make-A-Wish board, and is the medical director for Special Olympics Idaho. She enjoys skiing, waterskiing, travel, and time with her husband and two sons.

Kirk J Lewis, MD

Kirk J Lewis
Dr. Lewis received his undergraduate and medical degrees at the University of Michigan, where he was a two-year captain for Michigan's football team. He completed his orthopedic surgery residency and fellowship in sports medicine at UCLA. He also sought additional fellowship training in orthopedic trauma in St Gallen, Switzerland, and pediatric orthopedics at the University of California, San Francisco. Dr Lewis joined his long time friend and colleague Dr. Wade and the Idaho Sports Medicine Institute in 1985.
Dr. Lewis took a leave from ISMI from 2007-2011 where he was an Associate Professor at the University of California Davis. He was involved in education with orthopedic residents and their sports medicine fellowship program. He was also a team physician for the Sacramento Kings and the Sacramento Monarchs. Dr. Lewis has a strong interest and experience in global health and medical volunteerism.

Linda Hammann, MS, PT, SCS, ATC, LAT
Linda earned her bachelor’s degree in physical therapy from Northern Arizona University in 1982. She has worked at the Idaho Sports Medicine Institute since 1991, and is currently the Director of Sports Rehab.
Linda is also an NATA certified athletic trainer, earning her ATC in 1991 through a fellowship at HealthSouth and an internship program at Boise State University. She is a Board-certified Specialist in sports physical therapy, having passed her Sports Certification Specialty examination for the APTA in 1996.
In 1996, Linda was a volunteer for the 1996 Paralympics Summer Games, and in 1997 was selected by the USOC to work as an athletic trainer for the US track team at the World University Games in Sicily. She was also selected by the USOC as athletic trainer for the US Figure Skating Team during the 1998 Winter Olympic Games in Nagano, Japan. After that, she volunteered for the US Figure Skating Association as team physiotherapist for competitions in Halifax, Nova Scotia and in Oberstdorf, Germany.
Here in Boise, Linda spent 5 years providing volunteer athletic training services for the USARFU, working and traveling for with the local rugby team, the Snake River Snakes. She also volunteered for the Ore-Ida (HP/Power Bar) Women’s Challenge Bicycle Race for 4 years, traveling with the tour as part of the medical support team.
In her tenure in Idaho, Linda has been an avid kayaker, backcountry skier, runner, bicyclist, and hiker. She currently is involved with equestrian sports, and spends most of her free time with her horses, caring for them and pursuing the disciplines of dressage, stadium-, and cross-country jumping.
At ISMI, Linda is extensively involved in rehab, treating patients of all types, working with sports medicine fellows, and doing administrative work as well.

James R. Moore, MS
Jim graduated from the University of Colorado with a master’s of science degree in Exercise Physiology. He started working with Dr. Wade in 1979, and helped build Idaho Sports Medicine Institute into what it is today. Jim currently is the director of Athletic Testing and Performance. He provides VO2 max, isokinetic strength, anaerobic power and body composition testing for area athletes. He also designs and implements strength and conditioning programs for athletes of all skill levels. Jim has served as adjunct faculty with the Kinesiology department at BSU since 1985. He is extensively involved with both students and faculty as a research advisor. Most recently, he developed and taught a graduate level sports nutrition course for BSU. His other areas of interest include: hiking, mountain biking, and keeping fit.

Korene Mayo, ATC, LAT
Korene Mayo, ATC, LAT
Korene graduated from Boise State University with a bachelor’s degree in Athletic Training in 1984, and became certified later that year. She has been an integral part of the sports therapy team at ISMI since 1984. Her rehabilitation specialties include: running gait assessment, foot and ankle rehab, and advanced athletic return to sport techniques. Korene has also worked as the assistant Athletic Trainer at Timberline High School since 1999, where she works with women’s varsity sports teams. Her other areas of interest include: mountain biking, snow skiing, golf, tennis, snowshoeing and spending time with her daughter.

Brad Page, MS,MPT
Brad graduated with a BS from Boise State University in 1983. He completed a MS in Exercise Science at BSU in 1987, and a master’s degree in physical therapy at ISU in 1992. After PT school, he returned to BSU and started working at ISMI. Brad’s rehabilitation specialties include shoulder injuries and post-operative rehab. He is also a certified ASTYM clinician. Brad has taught as an adjunct faculty for BSU’s athletic training program. His other areas of interest include: alpine and Nordic skiing, triathlons, hiking, camping, and climbing peaks. Brad has been an active competitor in triathlons for the last 25 years.

Kristi Hulquist, MPT
Kristi graduated from the Army/Baylor Physical Therapy Program with a Master’s Degree in 1997. She served on active duty as a physical therapist at Ft Lewis, Washington for three and one half years, including one year as primary therapist for the Army ‘s elite Ranger unit. She came to ISMI in February of 2004 shortly after moving from Albuquerque, NM. She was certified as an ASTYM clinician in 2008. Kristi also coaches hurdles for the Centennial High School track team. Her other interests include: running, backpacking, travel and keeping up with her two children.

Beth Schoenfeld MPT, OCS, ATC
Beth Schoenfeld MPT, OCS, ATC
Beth graduated from the University of Northern Colorado with a Bachelor's Degree in Athletic Training in 1995 and became a certified athletic trainer. She graduated from the Army-Baylor Physical Therapy program with a Master’s Degree in 1997. She served on active duty in the U.S. Navy as a physical therapist for seven years at three different duty stations around the world. She is currently a Lieutenant Commander in the Navy reserve. Beth is a certified specialist in orthopedic physical therapy, has had additional training in manual therapy and is a certified ASTYM clinician. Beth came to ISMI in 2006 after moving to Boise from Jacksonville, Florida. She also teaches as adjunct faculty for BSU's Athletic Training program. Beth and her husband enjoy spending time in the outdoors, traveling, and chasing after their twins.

Jay Armstrong, PT
Jay graduated from The University of North Dakota with a Master’s Degree in Physical Therapy in 2001. He is a certified ASTYM practitioner and APTA credential clinical instructor. He is also a bicycle-fitting specialist and enjoys finding ways to help people continue to cycle with physical limitations. His rehabilitation interests include shoulder and lower extremity mechanics. When not working, Jay enjoys all outdoor activities Idaho has to offer.
